The City Clerk's Office is the custodian of all city records.  For a records request form click here.
Please print the form. When you have completed the request, please submit it using one of the following options:

  • Fax to (318) 872-0402
  • Deliver it in person to the Clerk's Office at City Hall, 705 Polk Street. 
  • Mail it to City Clerk's Office, PO Box 773, Mansfield, La 71052
  • Fees apply to pay for the cost of printing.
